Signing up for REDCap is as easy as 1, 2, 3!

Two . . .

Login to REDCap at https://redcap.health.utah.gov/redcap. You may get a notice saying that your access is denied, this is normal. We recommend that you bookmark the page. See detailed instructions for accessing REDCap.

Three . . .

Ask your supervisor to send an email to Carl Letamendi, cletamendi@utah.gov and Sri Bose, sbose@utah.gov indicating that you are authorized to submit patient safety reports on behalf of your facility.

Reporting an Adverse Event

Once you have signed up for an account. You are ready to submit your event using the REDCap system.

For Patient Safety Events: 

  1. Go to the REDCap Record Entry page.
  2. Click "Add New Record"
  3. Enter the information of the new event.
  4. Click ”Save and Exit Form” at bottom or top right of the page to report the event.

For Anesthesia Related Events

  1. Go to the REDCap Record Entry page.
  2. Click "Add New Record"
  3. Enter the information of the new event.
  4. Click ”Save and Exit Form” at bottom or top right of the page to report the event.
