Anesthesia and/or Sedation Adverse Event Reporting

The purpose and authority of Recording and Reporting Adverse Events from the Administration of Sedation or Anesthesia is depicted in R434-150. As defined in the Utah Administrative Code, an adverse event is a reportable event that includes the administration of sedation or anesthesia; in an outpatient, non-emergency room setting; that results in escalation of care, harm to, or rescue of the patient; and while under the direct care of the provider at the facility or within 24 hours of discharge.

Two . . .

Three . . .

Ask your supervisor to send an email to Carl Letamendi, and Sri Bose, indicating that you are authorized to submit patient safety reports on behalf of your facility.

Reporting an Adverse Event

Once you have signed up for an account. You are ready to submit your event using the REDCap system.

  1. Go to the REDCap Record Entry page.
  2. Click "Add New Record"
  3. Enter the information of the new event.
  4. Click ”Save and Exit Form” at bottom or top right of the page to report the event.
